Every school year, parents/guardians must log in to Skyward and give consent for Clinic Visits. This consent is extremely important because it allows our clinic staff to provide your student with basic care when needed, such as ice packs, Band-Aids, and bug bite lotion.

Please take a moment to log in to Skyward and make sure your Clinic Visit consent is completed for this school year.

• Click on Online Forms.
• Select and complete the Student Health Services form.
